The name for the exile is your name, it's part of the creativity in the game. He/she isn't assigned a name for that reason. Same goes with revan, you never actually hear his/her real name.

wow, toughie! Did you have anything running in the background when you were installing? If so, uninstall and re-install and in doing so turn of virus protection and any other thing that may compromise the installation. If that still doesn't work it may be a faulty disk, take it back to the business you bought it from and tell them you want a new one, or if possible I would recommend getting your money back. Purchase it from a different store to make sure that the other store didn't get a bad batch. If that doesn't work, if you have the money you could take it to a professional to see what they can do.
